1. What information do we collect, and how is it stored?

Personally Identifiable Information (PII)

In Short: We collect and store personally identifiable information that you voluntarily provide to us in a HIPAA-secured cloud service.

You may voluntarily provide PII to us via our website’s therapy inquiry form, which can be used to identify you personally and contact you. Our therapy inquiry form where we collect this information can be found at www.pasadenahealing.com/contact. The information we collect on this form includes:

  • First and last name

  • Telephone number

  • Email address

  • Reasons for seeking psychotherapy services

Your PII is securely stored in our Microsoft E365 E3 cloud, which is compliant with the Health Information Portability & Accountability Act’s (HIPAA) requirements for storing Protected Health Information (PHI).

 

Anonymized data automatically collected

In Short: Some anonymized information—such as usage, location, and tracking data—is collected automatically when you visit our website or engage with our marketing and stored by various third party platforms.

We automatically collect certain information when you visit our website or interact with our marketing. This information does not reveal your specific identity (like your name or contact information) but may include device and usage information, such as your: IP address, browser and device characteristics, operating system, language preferences, referring URLs, device name, country, location, information about how you navigate and interact with our website and/or marketing, and other technical information. This information is needed to maintain and improve the function of our website and marketing efforts, and it is generally stored by third party platforms such as Google Analytics, Google Tag Manager, and Google Ads. Personally Identifiable Information (PII)

In Short: We process your PII in order to respond to direct inquiries about our clinical services.

There are numerous ways we process your PII submitted through our website, as detailed below:

  • To process your PII for purposes of responding to therapy inquiries submitted through our website and facilitating the onboarding process when a person agrees to become a patient at Pasadena Healing Inc.

  • To respond to inquiries about our professional services, provide details about our practice policies, and resolve any potential issues you may encounter engaging with our website or marketing.

  • To facilitate account creation and authentication and otherwise manage user accounts via SimplePractice, our electronic health record, prior to completion of our Outpatient Services Agreement and initiation as a patient. We may process your information so you can create and log in to your account, as well as keep your account in working order.

  • To request feedback. We may process your information when necessary to request feedback and to contact you about your use of our website and/or marketing.

  • To save or protect an individual's vital interest. We may process your information when necessary to save or protect an individual’s vital interest, such as to prevent harm to themselves or another person.

In Short: We process your anonymized information to maintain and improve our website and marketing, address security and fraud prevention, and to comply with law.

We process your personal information for a variety of reasons, depending on how you interact with our website and marketing:

In Short: We do not share your PII with anyone except as explicitly authorized to do so by you.

The PII you provide to us via our website’s therapy inquiry form is stored securely, as described above, and is not shared with anyone without your written authorization. Please note, once a person becomes a patient of Pasadena Healing Inc, they review and agree to our Outpatient Psychotherapy Services Agreement, which includes an acknowledgement of our right to share PII for the purposes of preventing harm and coordinating payment for services.

In Short: We may share anonymized information in specific situations described in this section and/or with the following categories of third parties.

Vendors, Consultants, and Other Third party Service Providers. We may share your data with third party vendors, service providers, contractors, or agents (“third parties”) who perform services for us or on our behalf and require access to such information to do that work. In such cases, we will have contracts in place with our third parties that require them to help safeguard your information. This means that they cannot do anything with your information unless we have instructed them to do it. They will also not share your information with any organization apart from us. They also commit to protect the data they hold on our behalf and to retain it for the period we instruct. The categories of third parties we may share your information with are as follows:

Mobile Advertising

You can opt out of having your mobile advertising identifiers used for certain types of advertising by accessing the appropriate settings in your mobile device and following the instructions. If you opt-out, we will remove all data about you and will not collect any further data. The random ID previously assigned to you will be removed. Thus, if at a later stage, you decide to opt-in, we will be unable to continue and track you using the prior ID and you will for all practical purposes be a new user. For iOS devices, to limit interest-based ad tracking, go to Settings > Privacy > Advertising > Turn on “Limit Ad Tracking.” For Android devices, go to Settings > Google services > Ads > Turn on "Opt out of Ads Personalization." For iOS and Android, you can also reset the advertising identifier that is currently assigned to you. Do Not Track

Some Internet browsers, like Internet Explorer, Firefox, and Safari, include the ability to transmit "Do Not Track" or "DNT" signals. Since uniform standards for "DNT" signals have not been adopted, our site does not currently process or respond to "DNT" signals.

Flash Cookies

5. How long do we keep your information?

In Short: We keep your Personally Identifiable Information (PII) for a period of 7 years, after which it is deleted. Anonymized data that is automatically collected is kept for as long as necessary to fulfill the purposes outlined in this privacy notice unless otherwise required by law.

 

All information that could be considered PII or PHI will be kept securely for a period of 7 years in our HIPAA-secured cloud service, after which it will be deleted. We will only keep your anonymized data for as long as it is necessary for the purposes set out in this privacy notice, unless a longer retention period is required or permitted by law (such as tax, accounting, or other legal requirements).

 

6. Do we collect information from minors?

In Short: We do not knowingly collect data from or market to children under 18 years of age.

 

We do not knowingly solicit data from or market to children under 18 years of age. By using the website, you represent that you are at least 18 or that you are the parent or guardian of such a minor and consent to such minor dependent’s use of the website. If we learn that personal information from users less than 18 years of age has been collected, we will take reasonable measures to promptly delete such data from our records. California residents’ right to opt out of any “sale” of personal information under the California Consumer Privacy Act (CCPA):

In view of the definition of “sale” under the CCPA, Pasadena Healing Inc has the right, either now or in the future, to exchange, share, and/or “sell” (as defined under the CCPA) the following categories of personal information to certain third party data analytics providers: (1) personal identifiers; (2) characteristics of protected classifications under California and federal law; (3) Internet/electronic activity; (4) commercial information; and/or (5) inferences drawn from the categories described above in order to create a profile about you to reflect your preferences and characteristics. Our Company, however, chooses not to sell any personally identifiable information about site visitors or those who engage with our marketing.

Instructions if you want to explicitly opt out of such “sale” of personal data (even though we do not sell such information)

California Residents have the right under the CCPA to opt out of any “sale” of your personal information to third parties, as the term “sale” is defined under the CCPA. In Short: Yes, if you are a resident of California, you are granted specific rights regarding access to your personal information.

 

California Civil Code Section 1798.83, also known as the "Shine The Light" law, permits our users who are California residents to request and obtain from us, once a year and free of charge, information about categories of personal information (if any) we disclosed to third parties for direct marketing purposes and the names and addresses of all third parties with which we shared personal information in the immediately preceding calendar year. If you are under 18 years of age, reside in California, and have a registered account with an online service, you have the right to request removal of unwanted data that we have collected from you. 9. What is the Controlling the Assault of Non-Solicited Pornography and Marketing Act (CAN-SPAM)?

In Short: The CAN-SPAM Act establishes requirements for the way businesses can send unsolicited emails to you, including the right to opt out.

The CAN-SPAM Act is a United States law that sets the rules and penalties for commercial email messages, including those that promote content on websites or products. It covers all commercial messages, not just bulk email, and applies to some text messages sent to wireless devices. The act requires that any commercial email message sent include a valid physical postal address for the business. In Short: We do not currently respond to DNT browser signals.

At this stage no uniform technology standard for recognizing and implementing DNT signals has been finalized. As such, we do not currently respond to DNT browser signals or any other mechanism that automatically communicates your choice not to be tracked online. If a standard for online tracking is adopted that we must follow in the future, we will inform you about that practice in a revised version of this privacy notice.
